Schedule: Generally Monday through Friday between the hours of 8am-5pm, 30 hours/week that are flexible.

Job Summary:
The Family Connects Nurse Home Visitor (NHV) is a member of the Healthy Families team at Eagle County Public Health and Environment. This role works specifically with the Family Connects program but will collaborate across the Healthy Families team to achieve the goals of Eagle County Public Health and Environment and Eagle County Government. The NHV provides in-home visits for families across Eagle County, utilizing the evidence-based Family Connects model. The NHV collaborates with clinical and non-clinical staff to address system-level needs for families in the community.
Background on Family Connects:
The Family Connects model is an evidence-based and successfully demonstrated program that connects parents of newborns to the community resources they need through postpartum nurse home visits. The model was developed in 2008 through a partnership with Duke University, the Center for Child &Family Health (CCFH), and the Durham County Health Department. As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parentsโ needs, and helps to identify community services or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally.

Essential Functions:
- Follow the Family Connects home-visiting protocol. Keep records of client observations and activities as required. Document information for family assessments and program evaluation.
- Follow the Family Connects scheduling protocol. Visit target area hospitals as needed to schedule home visits for new mothers.
- Provide a health and physical assessment of mother and baby in the home to determine well-being.
- Assess family needs in 12 domains of physical and psychosocial well being, including depression, substance abuse, and domestic violence.
- Exercise clinical skills in appraising situations and making decisions.
- Establish a trusting relationship with families during in-home visits and engage the family to establish a family support plan with goals, objectives, and activities that address family strengths and needs.
- Work with the team to ensure appropriate referrals and connections to community resources as needed. Work in a team to assist parents in making and attending appointments for doctors, social service agencies, etc. Act as a liaison between families and other community agencies, serving as family advocate as required.
- Apply working knowledge of parent-child interaction, child-maternal health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assist in creating a health plan for entire family.
- Maintain eight open visit slots and conduct and document at least six home visits per week, depending on FTE.
- Work cooperatively as an active member of the Family Connects team by attending weekly team meetings, clinical supervision, staff meetings, and other meetings as required. Support and encourage development of other staff and of the Family Connects program.
- Use a health equity and trauma-informed lens in all work. Act as a public health role model to exemplify the principles of social justice, increase cultural competence, and work to close gaps in health outcomes that impact populations disproportionately.
- Observe safety and security procedures, use equipment and materials safely and report potentially unsafe conditions.
- Collaborates with the Healthy Families team to provide additional programming support as necessary based on community needs.
- Programming will focus on the perinatal population and be tied to the Public Health priorities of Increasing Social Connectedness and Increasing Access and Quality of Physical and Mental Health Care.
- Serve the community by demonstrating Eagle County Government mission and core values and following relevant policies and procedures.
- Other duties as assigned, including support of public health measures benefiting the community at large.
Qualifications:
Education, Experience, Licensure, Certification, or Registration:
- Bachelorโs degree in nursing or Associateโs degree in nursing is required.
Current Registered Nurse licensure to practice in the State of Colorado is required. Nurses with compact licensure must meet all licensure requirements set forth by the Colorado Board of Nursing within ninety (90) days of employment. - Three years of recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred OR any combination of education, training and nursing experience to meet the needs of the position.
- Current CPR certification is required.
- Valid driverโs license and satisfactory driving record is required.
Knowledge, Skills and Abilities:
- Ability to read, write, speak and understand English is required. The ability to read, write, speak and understand Spanish is preferred.
- Ability to express ideas and information clearly and concisely in a manner appropriate to the audience both verbally and in writing.
- Ability to provide services to assigned caseload and complete paperwork in a timely manner.
- Ability to stay current with basic computer skills.
- Demonstrated skill in working together with others through idea-sharing and collaboration.
- Ability to perform basic physical and mental health assessments.
- Ability to prepare and maintain accurate and neat records.
- Self-motivation and ability to work with minimal supervision.
