Position Summary
As a Community Health Advocate (CHA), you will be the bridge between individuals and the resources they need to thrive. You will empower families, strengthen community connections, and promote healthier lives across Eagle County. Through care management, outreach, and advocacy, you will help patients navigate services, adopt healthy behaviors, and access the support they need.

Essential Duties & Responsibilities

Patient Support & Advocacy
Build trusting relationships with patients and families, offering encouragement and emotional support
Act as a liaison between patients, providers, and community agencies
Provide ongoing follow-up, and basic motivational interviewing with patients/families
Help patients set personal health goals and attend appointments
Keep a good understanding of the comprehensive resources offered in the community, to be able to share them appropriately with community members.

Care Coordination & Communication
Provide referrals to community services and connect patients with transportation resources
Maintain consistent communication with providers and document patient progress in the appropriate systems
Collaborate with clinical staff to ensure comprehensive care plans
Maintain HIPPA compliance at all times
Assist with managing the MIRA phone line, and the main MIRA email

Community Engagement
Represent and communicate the mission of The MIRA Bus
Participate in outreach events, trainings, and staff meetings
Engage with current and potential new partners to secure services and resources to be offered on the bus
Promote awareness of available resources and services

Other duties:

Assist with keeping record of every community member that comes to the bus, to keep accurate data of resources and services provided.

Qualifications
High school diploma or equivalent 
MT, Paramedic, or CNA certification preferred
Completion of a Community Health Worker training program preferred (training available at no cost)
Medical terminology or background preferred
Written and oral fluency in English and Spanish required
Experience in multicultural and community-based settings preferred (1–2 years)
Strong organizational, and communication skills
Have great customer service skills to communicate with and serve others
Ability to work independently, prioritize tasks, and manage caseloads
Knowledge of local community agencies and resources
Basic computer skills; Electronic Medical Record experience preferred
